Thailand Inventory Nomination: How To Create An Account

Chemical nomination for inclusion into the Thailand existing chemicals inventory will end soon. However, some enterprises haven’t started the notification because they haven’t got accounts on DIW’s website. Account creation is not as convenient and fast as many thinks. ChemLinked would like to share with our readers the process and required documents for creating an account.

1. The importer or manufacturer should apply for a request form (สว. อ 2) at: http://haz3.diw.go.th/hazvk/jsp/master/register.jsp and print it out and sign it with company stamp.

2. Submit all the required documents to DIW via email or post mail. The email address will be given when the above application is completed.

The required documents include:

1. Form สว. อ 2.

2. Copy of VAT certification (ภพ 20) signed off with company stamp.

3. Copy of company certification (not more than 6 months) signed off with company stamp.

4. Copy of ID of the registrant with signature.
